Over Projected or Pinocchio Nose

In this type of nose, the tip projects too far forward in front of the face possibly due to overgrowth of the tip cartilages or overgrowth of the septum that the tip cartilages are attached to. This is clearly more noticeable from the side view. If the septum has overgrown, pushing the tip out in front of it, this is called a tension nose.  In a surgical plan one has to decide if only the tip needs to be set back or possibly the tip and the bridge of the nose as well.  As one make a surgical plan, he also has to think about consequences. Sometimes when the tip projects too far forward and is set back, the nostrils flare out so more nostril opening is seen from in front. If this actually happens, the flaring of the nostril can be reduced by a small skin excision at the same time with the resulting scar often partially inside the nose.
